I, hereby acknowledge any and all allied agencies and related individuals who have provided services to me at Odyssey House, New Roads Behavioral Health Services, Family Life Center to exchange information about my participation in the PRIME for Life® class .This includes copies of my educational records pertaining to the PRIME for Life® class at Odyssey House, New Roads Behavioral Health Services, Family Life Center. Odyssey House, New Roads Behavioral Health Services, Family Life Center will use this information in providing substance use educational care to me.
I specifically intend and affirm that this authorization extends to and includes information regarding drug or alcohol treatment/education services provided by programs governed by 42 CFR Part 2, rules providing federal protection for the privacy of my drug and alcohol treatment history.
I intend this authorization to remain in effect until 30 days after the last service I receive at Odyssey House, New Roads Behavioral Health Services, Family Life Center, unless I revoke the authorization before that time. I may revoke this authorization at any time by informing Odyssey House, New Roads Behavioral Health Services, Family Life Center in writing. I acknowledge that any information disclosed before I revoke this authorization may be subject to use and possible re-disclosure.
I acknowledge that Odyssey House, New Roads Behavioral Health Services, Family Life Center has affirmed that my treatment/education is not, and may not be, conditioned on my agreement to sign this authorization.
I acknowledge that Odyssey House, New Roads Behavioral Health Services, Family Life Center has offered a copy of this authorization to me for my records.
